Registration for this event is being conducted by the Community Health Care Association of New York State (CHCANYS). 


Telehealth is emerging as a critical component for increasing access to care, promoting cost effective service delivery, and establishing connections with specialty or limited providers. As leaders in providing high-quality, accessible primary care, community-based providers are exploring the use of telehealth to meet the challenges of serving diverse patient populations with complex needs.

This two-day training event, presented by CHCANYS in partnership with Finger Lakes Community Health, FTLF, and the National Association (NACHC), will provide attendees with an array of information and resources for developing telehealth capacity in the community-based setting. The agenda includes in-depth discussions with industry experts, and live simulations with telehealth equipment. Additional areas of focus will cover telehealth and the Triple Aim, legal and liability considerations for community-based providers, strategies for program implementation and integration, telehealth policy in the evolving healthcare landscape, vendor relations, and potential partnerships.

From NYC (East Side): North on FDR to Willis Avenue Bridge into Major Deegan expressway. From Major Deegan Expressway to New York Thruway I 87 (North). Take exit 9 Tarrytown (Last exit before the Tappan Zee Bridge). At the end of the exit ramp turn left onto Route 119 West. Continue to the end of the road and turn left at the traffic light onto Route 9 South. At the 1st traffic light turn right into hotel parking lot.

From NYC (West Side) North on West Side Highway to Saw Mill River Parkway to Exit 20(I 287 West-Tappan Zee Bridge) to New York State Thruway I 87(North). CARRIE BILL RILEY (FTLF)

Carrie Bill Riley is a Partner in the health law, non-profit and corporate law, and federal grants law practice groups. She counsels a wide variety of health care clients, including Federally Qualified Health Centers and other private and public health care providers, on contracting, regulatory compliance, fraud and abuse, and reimbursement matters. Carrie also provides counsel on  transactional matters, including formation and agreements for multi-provider affiliations/joint ventures, general contracting, and residency training arrangements. MARY ZELAZNY (FINGER LAKES COMMUNITY HEALTH)

Mary Zelazny began her career in 1989 at Finger Lakes Community Health as a Community Health Worker, expanding into several roles within the organization, including work in program development, finance and administration. Appointed as CEO in 2006, Mary continued to seek opportunities to build on the successes of this growing FQHC. During Mary’s tenure, Finger Lakes Community Health has opened 7 additional health center sites, as well as expanding FLCH’s Migrant Voucher Program into 42 counties of NYS. As the leader of an organization with PCMH Level III 2014 recognition, Mary has promoted the incorporation of a high level of cultural competency of staff, as well as integrating care coordination and technology into primary care that has created new collaborative relationships. This effort has resulted in expanded access and better health outcomes for patients by addressing the many barriers to care that are inherent in rural communities of New York State.

SIRENE GARCIA (FINGER LAKES COMMUNITY HEALTH)

Sirene Garcia earned her Masters of Science Management degree from Keuka College. Sirene has been working in Telehealth since 2010. Sirene played a pivotal role in helping to bring on new partners and to work with a variety of individuals and agencies that were interested in using technology to expand services. Her work continues today as the Director of Special Programs for Finger Lakes Community Health.

CASEY CASTNER (FINGER LAKES COMMUNITY HEALTH)

Casey Castner has been working in Telehealth since 2016. Through her role, she has learned the intricacies of the field, including equipment, IT, clinical aspects, billing and regulatory, and daily operations. She continues to work with Finger Lakes Community Health as the Telehealth Operations Coordinator.

OSBELIA JIMENEZ (FINGER LAKES COMMUNITY HEALTH)

Ozzy graduated from Utica College in 2004 with a Bachelor of Science in Nursing Degree and started working with Finger Lakes Community Health in September of 2004 as a Registered Nurse. In December of 2018 she accepted the position of Telehealth Clinical Coordinator. She brings with her all the knowledge and experience of working in multiple positions, from case management, providing in-camp services, floor RN, RN care pod leader, transition of care coordinator, in-office “NYSIIS” expert, certified medical interpreter, certified telemedicine clinical presenter, and certified retinopathy photographer. Ozzy has always been a leader and pioneer in many new programs and embraced telehealth when it 1st started as a program. She brings with her new ideas, passion, energy, and a love for Telehealth.


SUSAN SUMRELL (NACHC)

Susan Sumrell is the Deputy Director of State Affairs at the National Association of Community Health Centers (NACHC). Susan’s responsibilities include reviewing and analyzing federal and state regulations and policies affecting health centers, and advising health centers, primary care associations, and NACHC staff on these issues. Her main focus area is health center reimbursement policies, specifically Medicare, Medicaid, and CHIP. Prior to joining NACHC, Susan worked on Capitol Hill.


SHANNON MCDEVITT (HRSA)

Shannon McDevitt, MD, MPH is a board certified family physician with expertise in primary care, practice transformation, workforce development, clinical quality improvement, and population health. She currently serves as a physician in the Office of Policy and Program Development in the Bureau of Primary Health Care at the Health Resources and Services Administration (HRSA). In her roles at HRSA, Dr. McDevitt has developed and led the administration of funding opportunities, including responding to Administrative priorities such as the opioid epidemic, health information technology, integrated practice models, and precision medicine that support innovative, effective primary care. She also supports a variety of department and agency-wide activities, including supporting the development of Healthy People 2030 and the implementation of the HRSA telehealth strategic plan. Dr. McDevitt received her Doctor of Medicine degree from Wayne State University School of Medicine and her Master in Public Health from the University of Pittsburgh. She completed family medicine residency training at Moses Cone Memorial Hospital and a faculty development fellowship at the University of Pittsburgh Medical Center-St. Margaret.


SANDEEP KRISHNAN (MEDTEC INTERLINX, LLC)

Sandeep Krishnan is the President of MedTec InterLinx, LLC – a healthcare technology consulting firm based in Rochester, NY, working with healthcare organizations on projects focused on improving healthcare access and enhancing organizational efficiencies and cost improvements by use of healthcare information technology. Mr. Krishnan has over 20 years’ experience in the areas of telehealth and distance-learning systems implementation, video-conferencing technologies, operating room systems integration and wireless and broadband systems design, development, and integration. Mr. Krishnan‘s career has included business development and consulting, program management, and project implementation positions at GTE Corporation, Frontier Corporation and Stryker Corporation. At Stryker, he was responsible for the implementation of Minimally-Invasive Operating Rooms at leading hospitals in northeastern United States.

He has Master of Business Administration (MBA) degree in Marketing & Information Systems from the University of Rochester (2002) and a Master of Science (MS) degree in Electrical Engineering from the University of South Florida (1993). He has professional involvement with the American Telemedicine Association (ATA), Healthcare Information and Management Systems Society (HIMSS), and the Office of HIT at the NY Department of Health.


DANIELLE LOUDER (NORTHEAST TELEHEALTH RESOURCE CENTER)

Danielle Louder is Program Director for the federally funded (HRSA/FORHP) Northeast Telehealth Resource Center (NETRC), a member of the National Consortium of Telehealth Resource Centers, where she leads efforts focused on implementation and growth of telehealth programs throughout the NETRC region, (New England, NY and NJ), as well as collaborating with colleagues from the National Consortium on nation-wide efforts to advance the reach and impact of telehealth. Regional activities are carried out in partnership with the University of Vermont’s Telemedicine Program, and through collaboration with many additional stakeholders throughout the region. Key efforts include working with stakeholders to advance access and improve health outcomes for rural and underserved populations through innovative models, (e.g. Project ECHO®), and assisting health systems and providers leverage telehealth to address today’s crucial health issues, (e.g. the opioid crisis).

Ms. Louder also serves as Co-Director of MCD Public Health, a member of the National Network of Public Health Institutes, and is responsible for maintenance/growth of the public health portfolio, identifying opportunities for funding diversity, fostering growth/capacity within the PH team, developing new initiatives and facilitating collaboration with organizations locally, regionally and nationally. In this role, she is responsible for expanding the range of public health programs, with emphasis on bridging population health and primary care as well as initiatives deploying technology to enhance health promotion and disease prevention. She has a breadth of experience working across multiple sectors, including health systems, public and community health, with content expertise in telehealth, technology, healthcare innovation, cardiovascular health, diabetes, and worksite health/wellness.


JORGE CUADROS (EYEPACS)

Dr. Jorge Cuadros is the founder of EyePACS, a telemedicine program used for detecting diabetic retinopathy in over 1000 community health centers providing over 700,000 retinal exams to patients with limited access to eye care. He leads the development of the software and clinical collaboration necessary for EyePACS to provide screening in diverse locations in the US and internationally. He is co-author and co-editor of two books, “Teleophthalmology” and “Digital Teleretinal Screening”, and has been conducting clinical trials on innovative technologies, such as artificial intelligence, laser imaging devices, and primary care information systems, for expanding access to diabetic eye care in underserved communities. He is currently Assistant Clinical Professor and Director of Informatics Research at the University of California in Berkeley, Optometric Eye Center.


ÁGI ERICKSON (WEITZMAN INSTITUTE)

Ági Erickson is the Director of Business and Partnership Development at the Community Health Center, Inc’s Weitzman Institute. Ági has overseen the replication and growth of CHCI’s unique Project ECHO since 2012. She has presided over the rapid growth and spread of this program to over 57 health centers supporting over 275 providers across the US and Europe. Ági has ten years of project management experience at CHC.

Ági completed Clinical Microsystems training at the Dartmouth Institute and is a certified coach. She is currently coaching a School Based Health Center Interdisciplinary Clinical Microsystem* Team focusing on health system improvement work specifically on the design and redesign of small, functional clinical units for a seamless, patient-centered, high quality, safe, and efficient health system. Ági received training in advanced facilitation by Ingrid Bens, an internationally recognized expert in facilitation skills, and completed lean training at Denver Health Lean Academy. She earned her Master’s Degree from Quinnipiac University in Organizational Leadership concentrating in Health Care Management.

What is CHCANYS Cancellation policy?

All cancellations received before February 22, 2019  will result in a full refund minus a $50 processing fee.
