Care Management Services in Medicare

Under Medicare, federally-qualified health centers (FQHCs) are authorized to furnish a variety of care management services, including:

  • Transitional care management (TCM)
  • Chronic care management (CCM)
  • Behavioral health integration (BHI)
  • Psychiatric collaborative care model (CoCM)

These services allow FQHCs to improve the quality and cost-effectiveness of services to Medicare patients using strategies such as comprehensive care planning and the management of transitions between health care settings. With the exception of TCM, Medicare pays health centers a fixed monthly fee for each of the care management services.

This webinar will provide an overview of the care management services covered under Medicare, including discussion of the program requirements, patient eligibility criteria, and billing requirements associated with each. We will also touch on the requirement of patient consent and the collection of coinsurance for care management services.

She is experienced in consulting on professional revenue cycle analysis and enhancement, medical practice operations, patient flow redesign, business process improvement and development, staffing analysis and redesign, practice efficiency and productivity and educational sessions and training. Certificates of Attendance: We verify attendance upon completion of a webinar (live or recorded version) and will only issue certificates in the name of the account holder enrolled in the course.
